Assessment of care need and client placement activities (SAS working group)
The goal when assessing care need and scaling care is to place the client in a location that corresponds to his/her functional capacity and need for assistance; however, so that home care is always the primary option. When it is no longer possible to manage at home, a care place corresponding to the care need will be arranged, either in an assisted service home or a home for the elderly.
The need for a care place is determined based on an assessment of the client´s functional capacity, which is performed by employees in home care and/or the 24-hour unit. Joint criteria and functional capacity assessment methods are used when assessing the need for care. The need for care is assessed by the SAS working group in the order of urgency. SAS is an acronym for the Finnish words selvitä (clarify), arvioi (assess) and sijoita (place). A working group consisting of the chief physician for geriatric care, two CAP co-ordinators and the service counsellor, who serves as the secretary is responsible for the activities.
